Millicent is one of the most important NPCs in Elden Ring. The one-armed lady will accompany you through half the game - if you find her. In this guide, we'll show you where to meet her for the first time and how to get her powerful talisman as well as one of the strongest weapons in the game.

How do I start the Millicent quest in Elden Ring?

To start the Millicent quest, the first thing you need to do is meet the NPC Sage Gowry. He is sitting in a small hut in Caelid, guarded by a wild dog with a giant skull. Kill the dog and talk to Gowry. He asks you to help a woman named Millicent. To do this, you will need the Pure Gold Needle, which is said to be hidden in the Scarlet Swamp of Aeonia. In return you will receive the note "Sellia's Secret".

Elden Ring Millicent Quest: Find the Golden Needle

Unfortunately, the pure gold needle is not just lying around in the swamp, but is guarded by a powerful boss: Commander O'Neil. He waits in a fringe area of the swamp in the heart of Aeolia. During the battle, he summons two large waves of spirit banishers. Use the spectral steed for the fight and patiently take them out one by one before turning your attention to the commander.

Afterwards, take the golden needle to Gowry. He will take it to repair it and ask you to come back later. Save and reload the game or rest in a place of grace and let some time pass. Afterwards, return to Gowry and get the needle. You can also solve the mystery of Sellia in the meantime.

Elden Ring Millicent Quest: Open the Gate of Sellia

Sellia, City of Wizards is located between Gowry's hut and the Church of Plague, where Millicent is staying. Read Gowry's note and you'll learn that in order to cross the ghost town, you need to ignite three hidden flames. Only then will the magic seal on the gate fall and you can continue your journey. Heal Millicent and get Talisman

If you have the pure golden needle got back from Gowry and broken the seal of the city gate of Sellia, ride up the mountain to the Church of the Plague. Murmur the beetle creatures at the door and talk to Milicent. Hand her the Pure Golden Needle and she will use it to try to rid herself of the Scarlet Blight.

Rest again in a place of grace and let some time pass. Milicent is now healthy again and presents you with the talisman Heir of the Prosthetic Wearer in thanks, which increases the attribute Skill by a whopping five points. Then return to Gowry's hut. If Milicent isn't there yet, let some time pass again in a place of grace. After the conversation in Gowry's hut, Millicent will leave on her journey and you will meet her again in later sections.

Where can I find Millicent's prosthetic in Shadow Castle?

Next, you will need to travel to the Altus Plateau area to the Lookout Mound at the Earth Tree. From the Mercy Site, walk north up the hill where there is a signpost statue. Halfway up, you'll meet Millicent. In conversation, she makes a comment about wanting a prosthetic arm. You will find it in the Shadow Castle high up in the north of the Altus Plateau.

Before you enter the castle, protect yourself against poison and fire damage. The entire courtyard is drenched in a poison lake - don't roll here. This will increase the effect of the poison. The zombies in the castle can also infect you with death plague, which will kill you instantly. Better keep your distance. Fight your way through until you meet a strong soldier in the northwest wing. He guards a room in which there is a treasure chest. Loot it and bring the Valkyrie prosthesis to Millicent.

Defeat the Apostle of the Scalps of the Gods and collect the Scalps of the Gods

The next stop is the Windmill Hill in the north of the Atlas Plateau. Pass through the windmill village with its insane inhabitants. At the top of the hill by the large windmill, you'll be met by the boss Apostle of the Scalps of the Gods. The nimble fighter performs some quick combos with his double blade, but you can easily dodge them with the right timing.

Watch out for the black flame. When he brings his hands to his chest, he quickly shoots them shortly after. Once you've reduced his life bar by about half, he'll start to elongate like Dhalsim in Street Fighter and perform circular blows. Dodge with pike jumps, get in close and hit him with charged up punches. The skeleton warrior's ghost ash is a useful helper, as it distracts the boss and keeps resurrecting.

After the boss fight, you'll receive the Double Blade Gods' Scalper and the Invocation Scourging Black Flame, as well as 14,000 runes. Go to the place of mercy. Afterwards, Millicent will stand in front of the windmill. Talk to her and she will offer you her services for upcoming battles.

Millicent at the mountaintop of the giants: Meet at the Ruins of the Primal Snow Valley

Millicent will be waiting for you next at the Mercy Place "Ruins of the Primordial Snow Valley" in the area "Mountaintop of the Giants". Talk to her to learn about the Halig Tree and a hidden medallion. It is said to be in a fortress to the north. Meet Millicent in Elphael, Support of the Halig Tree

Once you have fought your way through the Haligree and reached the place of grace "Prayer Room" in the area Elphael, Support of the Haligree, Millicent awaits you there. To do this, you must first defeat the boss Loretta, Knight of the Haligree defeat on the promenade of the Halig tree and then follow the main path. Millicent will stay in the prayer room until you kill the tree spirit with boils.

You will find the mini-boss if you go right over the root towards the rotting pool just before the place of mercy "sewer". The monster will pop up as soon as you set foot in the foul slop. Equip yourself with preserving boli to mitigate the effects of the scarlet rot, and watch out for the monster's attacks to push you over the cliff.

Last meeting with Millicent and both ends of the quest

After the fight, two call signs appear. With the golden one you help Millicent, with the red one you fight against her. Your choice has the following effects:

Follow the call to help Millicent: Choose the golden call sign and you will fight alongside Millicent against her four sisters. You will receive the Rotting Winged Sword Insignia (Talisman). Millicent lies on the opposite shore after the fight. Talk to her until all dialogue options are exhausted and return here after a short rest in a place of grace. You will now find the Pure Gold Pin.
Follow the call to challenge Millicent: Select the red call sign and you will fight Millicent. As a reward, you will receive the talisman Millicent's Prosthesis. Travel to Gowry's hut in Caelid and you will find his Sound Pearl and the Herd's Canvas Talisman.

Once you've decided to help Millicent, next kill Boss Malenia (Guide) at the roots of the Halig Tree. She leaves behind a gigantic red flower. Insert the Pure Gold Needle into it and you will receive an Ancient Dragon Darkforge Stone and Miquella's Needle. This will allow you to free yourself from the Frenzied Flame, as long as you have touched the Three Fingers in the Cathedral of the Lost beforehand.
